SYSTEM OF RENDERING MEDICAL CARE FOR CHILDREN WITH PERINATAL DISEASES IN MOSCOW

Abstract
Severe ante- and intranatal hypoxia is one of the main reasons of perinatal morbidity and mortality in Russian Federation. Children with perinatal disorders are needed in complex recovery treatment. In this regard, a number of normative documents appeared for the implementation and development of institutions providing highly skilled and high technology assistance for this category of patients. The purpose of this study was to investigate the current aspects of medical care for children with perinatal disorders in the modern conditions and development of proposals for its improvement. To conduct the study we used the method of expert interviews, statistical-mathematical data processing, logical generalization and analysis. We designed questionnaire, which included questions about indicators of accessibility and quality of medical care. The proposals for improving the organization of medical care were developed based on the received data.

Abstract
Optimization of rendering of medical aid to rural population is a key aspect of the development of health of the Russian Federation in this connection the aim of this study was to assess the main results of a multidisciplinary outreach teams for the period 2009-2017 in the village of Vikulovo Tyumen region. The frequency of detection of various diseases by specialists of mobile teams, including cerebrovascular disease, is analyzed. The effectiveness of the work of field teams was demonstrated, which was manifested in the form of reduction of pain and morbidity due to diseases of the nervous, musculoskeletal systems and CVB.

QUALITY OF LIFE AS A CRITERION OF EFFICIENCY OF TREATMENT OF GLAUCOMA PATIENTS

Abstract
The quality of life, along with other indicators, can comprehensively characterize the effectiveness of the treatment, including patients with glaucoma. The aim of the study was to study the quality of life of glaucoma patients after treatment with antihypertensive drugs. Material and methods. The quality of life study was conducted in 135 patients with glaucoma before and after 6 months of antihypertensive therapy by means of a questionnaire survey of the National Eye Institute Visual Functioning Questionnaire (NEI-VfQ-25) Russified version. Data processing is performed on a computer using the program "Statistica 6.0". Research result. A reliable decrease in all indicators of quality of life before treatment was established. After 6 months of conservative therapy in patients with glaucoma all indicators of quality of life increased significantly. As much as possible, such an indicator as near vision has increased. After treatment, significantly increased the perception of distance vision, General health, decreased dependence on strangers, problems with movement, pain in the eye. Conclusion. Indicators of quality of life are an important criterion for the effectiveness of treatment of patients with glaucoma.

ANALYSIS OF GENDER PECULIARITIES OF AGE DYNAMICS OF FREQUENCY OF TREATMENTS FOR AMBULATORY MEDICAL ASSISTANCE FOR CAUSES INCLUDED IN SECOND CLASS ICD-10

Abstract
Purpose of the study: a comparative analysis of the frequency of requests for out-patient medical care for men and women of different ages for reasons included in Class II of ICD-10. Data were used on the causes of treatment of 78,000 male and female patients for one year in medical organizations. The increase in the frequency of calls for reasons included in the second class of ICD-10 ("Neoplasms") in women can be described very accurately by a second-order parabola. The increase in the frequency of treatment is determined by the causes associated with benign tumors, but the specific gravity of malignant tumors increases with age of the woman and most strongly in young age groups. Gender differences consist in the dynamics of the frequency of treatment for benign tumors, whereas the dynamics of the frequency of treatment for malignant tumors do not have gender differences. The proportion of applications for out-patient medical care for benign tumors in women is significantly higher than for men in all age periods.

SATISFACTION OF NURSES OF STATE MEDICAL INSTITUTIONS

Abstract
The problems of increasing the motivation of health workers are the most important function of health management. Without their solution, it is hardly possible to realistically improve the quality and culture of rendering medical care to the population, as well as to increase the efficiency of the activities of medical organizations and the industry as a whole, based on the rational use of financial, material and human resources. In the health care system, nursing staff is the most significant part of the workforce. The aim of the study was to study the degree of satisfaction of nurses in a typical medical institution of the state type. The assessment showed that 55% of respondents are satisfied with their work, 39% are not satisfied, 4% are at a loss to answer. In a public medical institution they are not satisfied with the key parameters of the organization of work.

Abstract
Analysis of the main classical marketing concepts has shown that in order to achieve a high level of efficiency, a comprehensive, generalizing approach to the organization of activities is necessary, taking into account the characteristics of consumer behavior, the definition of target groups of consumers, the structure of their needs, the social role of the medical institution, the formation of its positive reputation and high degree of patient loyalty. Management of organizations, including medical ones, in the 21st century requires a comprehensive approach not limited to classical marketing concepts.

Based on the analysis of primary morbidity of diseases of the eye and adnexa for 2012-2016 retrospective continuous method in large cities of Krasnodar Krai is established that disadvantaged areas are Anapa and Armavir, including the incidence of glaucoma and cataracts. Krasnodar branch of FGU MNTK "eye microsurgery them. Acad. S. N. Fedorov" taken managerial decisions and improve organizational and consultative medical care of the municipal ophthalmology service and the formation of mobile teams of ophthalmologists in cities.

Abstract
Palliative care is characterized as " a set of medical interventions aimed at getting rid of pain and relief of other severe manifestations of the disease, in order to improve the quality of life of terminally ill citizens with various diseases, including cancer, other severe pathology, at the stage of uncontrolled progression of the disease». We have analyzed the state and availability of palliative care to date, and studied the history of the issue, the state and development of this issue, both in the Russian Federation and in a number of foreign countries. Attention is paid to the issues of the regulatory framework regulating the provision of palliative care and the use of narcotic analgesics. As a result of the analysis, it can be concluded that , despite the considerable attention to palliative care, the question of its physical availability remains very important.

THE PROBLEM OF ROAD TRAFFIC INJURIES IN PATIENTS WITH OBSTRUCTIVE SLEEP APNEA

Abstract
Accidents on road transport cause huge material and moral damage to society and individual citizens. Road traffic injuries leads to the exclusion from the sphere of production of people of working age. Patients with obstructive sleep apnea syndrome experience severe daytime sleepiness, which significantly reduces efficiency, limits attention and memory, disrupts coordination of movements and slows down the reaction rate. The occurrence of these violations significantly increases the risk of road accidents. However, our country does not take into account the cause of "falling asleep at the wheel" when fixing an accident. In addition, obstructive sleep apnea syndrome is not included in the list of diseases that can serve as a basis for limiting the issuance of a driver's license. The mandatory somnological examination of drivers can significantly affect the reduction of socio-economic damage from road accidents and the solution of the problem of road traffic injuries in patients with night apnea.
