POTENTIAL YEARS OF LIFE LOST IN THE BURDEN OF MORTALITY OF THE KALUGA REGION

Abstract
Mortality is an important source of information on the Public Health Services effectiveness. The national mortality data are strongly associated with natural ageing processes and are not sensitive to mortality among youth. Potential Years of Life Lost (PYLL) reflects social and demographic significance of mortality burden. The aim of this study was to assess the burden of suicides in Kaluga Region in comparison with other external causes of death. It has been established that between 2003 and 2018 the overall burden of suicides was 69574 PYLL representing 3.67% of total burden of mortality over the period. The men to women PYLL ratio was 4.22:1. The most vulnerable age cohort was 20-34 years. There is a noticeable upward trend in male burden of suicides in the 55-59 age group and a downward trend in female burden in the 50-54 age cohort. The urban PYLL data were higher than rural, inspite higher suicide mortality rates in rural areas.

DEFECTS OF THE MANAGEMENT OF THE MEDICAL HEALTH PROVIDING IN THE COVID-19 PANDEMIC

Abstract
The Ministry of Health of the Russian Federation has published the Order of the Ministry of Health of the Russian Federation dated 03.19.2020 No. 198n "On the temporary procedure for organizing the work of medical organizations in order to implement measures to prevent and reduce risks the spread of the new coronavirus infection COVID-19", that contains strict requirements. The pandemic is characterized by a shortage of resources, which leads to a large number of defects in health care delivery. Many violations committed by medical organizations have been identified. At the same time, despite the identification of hundreds of violations, only a few of them entailed administrative and criminal liability. A similar situation has arisen due to the absence of subjective guilt of both the person directly providing medical care and the officials responsible for organizing it. The article provides several examples of typical situations that entailed responsibility.

MAIN TRENDS IN THE EVALUATION OF HEALTHCARE TECHNOLOGIES IN THE WORLD IN THE PERIOD FROM 1980 TO 2020: LITERATURE REVIEW

Abstract
The article considers the development of health technology assessment: it reflects the dynamics of the introduction the technology assessment agencies into the healthcare decision-making system from 1980 to 2020. It was established that during the study period 60 organizations were created in 31 countries of the world. An analysis of the dependence of expert structures on the level of domestic gross product revealed that in 75% of countries with low per capita domestic gross product, there are no organizations evaluating health technologies. At average and above average levels, 50 per cent or more countries have the agencies, and in some cases the functions are distributed among several organizations. The study also examined the evolution of the applied methods, it was found that initially many countries used cost minimization methods and introduced thresholds of the indicator of readiness to pay into the system of decision-making on financing, while during the studied period there is the change from rigid evaluation criteria to multidisciplinary evaluation approaches.

STATE GUARANTEES OF HEALTH CARE AND WORK STANDARDS IN HEALTHCARE

Abstract
The analysis of regulatory legal documents concerning the planning of the main indicators of the volume of medical care in outpatient and inpatient conditions, for various specialties and profiles of beds for 2020 and 2021. identified changes in individual profiles and innovations in the normative-planned indicators of the volume of medical care. Including for the first time, the volume of a number of diagnostic studies was established, which actualizes the issues of staffing the auxiliary treatment and diagnostic service. The erroneous provisions in the territorial program and provisions that contradict the approved departmental order on labor standards are highlighted. Calculations were made of the planned number of beds for the profiles of medical care of "obstetrics", "geriatrics" and "medical rehabilitation" in the Russian Federation and a comparison of their planned and actual number. It is shown that the planned indicators are characterized by sharp annual drops, and the dynamics of the actual provision of bed capacity develops smoothly with the preservation of the general trend in all years of observation. Thus, there is no coordination of planned and normative labor indicators, and health care practice develops independently of them, which indicates a deep crisis in the system of planning the scope of medical care.

DICTIONARY OF REPRODUCTIVE HEALTH: TO THE PROBLEM OF THE PURPOSE AND METHODOLOGY OF THE COMPILATION

Abstract
An accurate and unambiguous perception of the conceptual and categorical apparatus, a uniform understanding of the meaning of statements are the fundamental basis of productive scientific activity, especially in actively developing areas of scientific knowledge and practical activity, which include the sphere of reproductive health issues. The importance of such work, important in itself, has acquired a special force in connection with the need to implement the tasks of the National Project "Demography". The article presents the rationale for choosing the type, type and form of a Dictionary on reproductive Health, corresponding to its subject matter and target orientation, analyzes the lexicographic features, purpose and methodological approaches to compiling a thematic dictionary on reproductive health, justifies the constructive and user advantages of presenting this dictionary in electronic form.

THEORETICAL BASIS OF THE SYSTEM OF MOTIVATING AND STIMULATING CITIZENS TO HEALTH-PRESERVING BEHAVIOR

Abstract
The theoretical foundation of the system of motivating and stimulating citizens to health-preserving behavior is presented, this system is a set of measures to reduce the prevalence of known behavioral risk factors for non-communicable diseases, in order to reduce the incidence of such diseases, to preserve human, and, therefore, labor, and the economic potential of the country. In order to clarify and form a unified view of the described topic definitions from the standpoint of public health and health care are given for such terms as motive, motivation, motivation process, incentive, which are actively used at the present time. A need-oriented approach to motivation was formulated. It consists in satisfying the objective need for improving the health of the population through indirect satisfaction of the needs of individuals. The definition of the system of motivating and stimulating citizens to maintain a healthy lifestyle is given, the stages of actions for the successful creation of a system are defined, which purpose is to strengthen the effect of health-enhancing factors, and weaken the effect of negative factors by restructuring the dynamic stereotype of citizens.

WORKING STANDARDS FOR DOCTORS-STATISTICS AND MEDICAL STATISTICS

Abstract
The work of statisticians and medical statisticians is an important component of the medical organizations activities. A study of the dynamics of the actual availability of these personnel revealed an increase in the number of doctors amid a decrease in the number of medical statisticians. The analysis of the staffing of medical organizations with these positions in modern regulatory documents was carried out in comparison with the data indicated in the orders of the USSR Ministry of Health. The publication presents an analysis of the staffing and normative provision of medical organizations with the posts of physicians-statisticians and medical statisticians, identifies the main disputable provisions of the modern legal and regulatory framework for labor. At the same time, the main normative indicators for the analyzed positions and differences in the value of the standard are established depending on the type of institution and its significance and role in providing medical care to the population. The main controversial provisions in modern normative and legal documents on labor, consisting in different indicators and meters of labor standards, were identified and, when applied, lead to unequal load of statisticians and medical statisticians with the same amount of work. Thus, the number of beds for which one post of medical statistics is established in institutions of the same type varies from 80 to 300 beds, and for a number of institutions the position is established regardless of the capacity of the institution. A reasonable conclusion was made about the need to revise the labor standards of doctors of statisticians and medical statisticians.

CHILDREN'S INFECTIOUS RISKS TO RESIDENTIAL AREAS: HOW TO MINIMIZE THEM?

Abstract
Most of the time a modern person spends in closed rooms and is exposed to the adverse factors of living quarters, both at work and while in the premises of residential buildings. In this regard, human health largely depends on how environmentally safe the internal living environment of the premises is. Children are especially sensitive to external factors. Living quarters are becoming a place where children can spend up to 24 hours a day. At home, the upbringing, educational process is implemented, the child's leisure and nutrition takes place. The development of standards and recommendations for the disinfection of living quarters is necessary in order to reduce the risks to the health of children - which is an absolute priority of the state policy of the Russian Federation. The purpose of this work is an analytical review of data on minimization of infectious risks of residential areas for children's health. The article deals with the areas of infectious risk of residential premises; classification of modern disinfectants; the safety of disinfectants for children's health; analysis of regulatory approaches in the Russian Federation to ensuring hygiene in residential premises, including during the COVID-19 epidemic; justification of educational activities on hygiene of housing at the family level.

ABOUT THE STATE OF PREFERRED MEDICINAL PROVISION

Abstract
Preferential drug provision is one of the tools that ensure the availability of medical care to the population. The total number of citizens to whom the state partially or fully compensates for the costs of obtaining medicines is about 20% of the population of the Russian Federation. In Russia, based on the sources of funding, two main categories of beneficiaries have been formed: regional and federal, the state exercises its powers through various programs. Since the end of 2004 - the beginning of 2005, the implementation of the DLO program began. In 2008, part of the authority to organize preferential drug provision was transferred to the subjects of the Russian Federation. The article presents an analysis of the state of preferential drug provision. The authors considered in detail the issues of providing orphan (rare) medicines, studied the state and problems of all types of preferential drug provision. In conclusion, there are proposals to improve the system of preferential drug provision.

QUALITY ASSESSMENT OF MEDICAL CARE BY INTEGRAL INDEX

Abstract
The redundancy of the proposed indicators for assessing the same fact, as well as the heterogeneity of the proposed indicators (quantitative and qualitative, referring to different objects, etc.) require minimizing their number by developing some generalized (composite, integral) indicators. In addition, these indicators are required at various levels of management to identify problems in the managed system. This publication provides an overview of the methodological approaches found in scientific sources for defining complex assessments (“universal”, which can be used for all types and conditions of health care and “special”, considered for a specific profile of health care, level of management, etc.). Many authors propose integrated methods that take into account not only a set of indicators formed in a certain way, but also the importance of each of the indicators; in addition, it is proposed to take account all components of medical care (general, medical, social and economic efficiency).

SOCIOLOGY OF MEDICINE: ISSUES OF METHODOLOGY

Abstract
In the article, the authors tried to concentrate on the issues that, in their opinion, are of the practical importance for the organization and conduct of Sociological Research in Medicine. The role, place and types of sociological research that can be applied in the quality management system of medical care are considered. The problems and difficulties arising with their use in the field of medicine are identified. The advantages and disadvantages are considered in the context of quantitative and qualitative methodology. The main methods used in the sociology of medicine are considered: questioning, interviewing, observation, experiment and document analysis. The basic principles and procedures of specific sociological research are formulated. Depending on the nature of the goal and the tasks set, various types of specific sociological research (descriptive, analytical, etc.) were identified.

INFLUENCE OF THE LEVEL OF EDUCATION AS A SOCIAL DETERMINANT OF HEALTH ON THE PREVALENCE OF HEALTH-PRESERVING BEHAVIORS

Abstract
The purpose of the study: to study the influence of the level of education as a social determinant of health on the prevalence of health-saving behaviors in women of reproductive age. The data were obtained through an anonymous questionnaire based on the questionnaire "Research of Social Determinants of Health", which was finalized by the authors. A total of 566 non-pregnant women aged 18-49 years were included in the study group. The lower age limit was deliberately limited to 18 years, since female adolescents aged 15-17 years did not reach adulthood, despite the fact that this age range is included in the concept of "reproductive age". The first comparison group included 306 women with higher or incomplete higher education. The second comparison group included 260 women with secondary or specialized secondary education. Each of the groups described above was divided into four more subgroups based on age. The majority of women, regardless of their level of education, had a positive attitude to the principles of a healthy lifestyle. Women with a high level of education were significantly more likely to follow the principles of a healthy lifestyle on a permanent basis. With increasing age, the frequency of following the principles of a healthy lifestyle increases in the group of women with a high level of education, and in the group of women with a lower level of education, it decreases. The study of certain elements of a healthy lifestyle showed that the frequency of irregular alcohol consumption and smoking in the group of women of reproductive age is quite high, more than half of the women surveyed admitted this. Women with a lower level of education were more likely to admit that they were overweight. The level of education did not significantly affect the level of physical activity. A high level of physical activity was noted by about a third of women in both groups compared. With increasing age, the proportion of women with high and very high physical activity in the group of women with a high level of education increased, while in the group of women with a low level of education, on the contrary, decreased. The data obtained allow us to consider education as a social determinant of health, which affects the prevalence of health-saving behaviors in women of reproductive age.

MORPHOLOGICAL MUSEUM OF NORTHERN STATE MEDICAL UNIVERSITY (1932-2012)

Abstract
The article was prepared within the framework of the research work "State policy in the field of health protection and medical science in the Russian Empire and the USSR" (subprogram "Monitoring of the network of medical museums of the Russian Federation"). The authors devoted their research to one of the museums of the Northern State Medical University - the morphological (anatomical) museum, which has been leading its history since the early 30s of the twentieth century. The historical period considered by the authors is 1932-2012. The article shows the role of the heads of the department of anatomy in the development of the museum, names of the employees of the department who have invested many years of work in the creation and preservation of the museum exhibits. A detailed description of the museum's exposition fund, which is unique, is given. The article uses materials of the creator and first director of the Museum Complex of the Northern State Medical University, Honored Worker of the Higher School of Russia, Doctor of Historical Sciences, Professor Gennady Stepanovich Shchurov, who devoted a number of his works to the history of the northernmost medical university. It is shown that since 1973, the museum has become not only a training base for a medical university, other higher and secondary specialized educational institutions, but also a center for systematic educational and vocational guidance, promoting a healthy lifestyle. Over 1000 tourists visit the museum annually. The teachers of the department conduct more than 50 excursions per year.
