LABOUR STANDARDS OF THE SUPPORT SERVICE: PROBLEMS AND SOLUTIONS

Abstract
The publication presents the theoretical foundations of various ways of calculating the number of the auxiliary medical and diagnostic service medical workers positions according to staff standards, the volume of activity and labor standards per unit of work, based on timing. Restrictions in their application in practical health care are due to the incomplete and ambiguity of modern legal documents on labor, the lack of labor standards for a number of positions, instructions on the shift of work as an indicator and meter of numbers without determining the number of shifts per number of outpatient doctors or number of beds. An analysis of the Russian Ministry of Health on the Regulations on the Organization of Primary Health Care, on the Rules for Research leads to the conclusion that there are current problems in the labor rationing system in healthcare and its management. The authors outlined the main directions for changing the current negative situation in the rationing of the work of medical workers of the auxiliary service, consisting of the need to create a Center for Labor Rationing, responsible for jointly with clinicians developing regulatory documents for application in medical organizations; a complete review of the entire labour legal framework; inclusion of labor rationing issues in the program of diploma and postgraduate training of doctors, medical workers with secondary education.

DYNAMICS OF THE GENETIC BURDEN OF PUBLIC HEALTH DUE TO CONGENITAL DEVELOPMENTAL ANOMALIES, DEFORMITIES AND CHROMOSOMAL ABNORMALITIES IN THE CONSTITUENT ENTITIES OF THE RUSSIAN FEDERATION FOR 2013-2019

Abstract
Congenital malformations, deformations and chromosomal abnormalities lead to either lethal or sublethal consequences. An analysis of the frequency of congenital malformations and chromosomal abnormalities can characterize the contribution of the genetic load to the state of this indicator and determine the state of the genetic health of a population of the Russian Federation. The aim of the research was to study the contribution of the genetic load at the level of populations of the constituent entities of the Russian Federation to the formation of indicators of public health and the subjective differences in its trends in recent years. The study was conducted by the method of continuous observation according to official statistics of populations of the constituent entities of the Russian Federation for 2013-2019. The results of the study showed that there are population differences in the contribution of the genetic load due to congenital malformations and chromosomal abnormalities. At the same time, it was noted that in the presence of a general pattern of formation of trends in increasing chromosomal abnormalities and congenital developmental abnormalities in all the constituent entities of the Russian Federation, there are significant regional differences in the rate of growth of the indicator for this class, especially in the dynamics of recent years, which must be taken into account when planning measures to improve public health including its genetic determinants.

FAMILY FACTORS IN SMOKING INITIATION AMONG MINORS

Abstract
The article is devoted to the actual problem of the health of children and adolescents. Prevention of tobacco consumption occupies a special place in a wide range of intensively studied reserves of health preservation of children and adolescents. An important protective factor against self-destructive behavior in adolescents is parental attention. The purpose of the study: the article proves family priority in protection minors from nicotine addiction formation. Survey results reveal environment peculiarities of teenagers that smoke in Russia. Smoking shows leading role of parents as an example for children and their close relationship with child to avoid unhealthy habits.

RECOGNITION OF RESPONSIBILITY FOR ONE'S HEALTH AS A DETERMINANT OF HEALTH IN OLDER AGE GROUPS

Abstract
The purpose of the study: to analyze the prevalence of the fact of recognition of responsibility for their health in older age groups. The results of an anonymous survey of 1045 people aged 60 years and older were analyzed. The results of this study lead to the conclusion that the largest share, both men and women recognize the fact of responsibility for their health, is seen in the age group of 60-64 years, with increasing age the proportion, which imposes the primary responsibility for the health of the state and the health care system, that is passive, increases. There are no gender differences in trends, but women are generally more responsible about their health, less likely to shift responsibility to the state and the health system. The level of education does not affect the value of the proportion of older age groups who recognize the fact of responsibility for their health.

MONITORING AS A N INDICATOR OF QUALITY IN THE SYSTEM OF CIRCULATION OF MEDICINES

Abstract
According to the World Health Organization, in the world, adverse drug reactions cause hospitalization of up to 20% of patients, and up to 15-20% of the health budget is spent on problems related to medicines. In this regard, due to the adoption of effective management decisions in the framework of the implementation of the instructions of the country's leadership to provide the population with affordable, effective and high-quality medicines, it is necessary to use information technologies, constant monitoring in order to manage the project, make adjustments if necessary, receive feedback, document activities and, as a result, provide guarantees for achieving the planned result. All of the above has served as an impetus to explore the use of an effective assessment tool, which is the monitoring of major programs in the field of drug circulation. The publication defines the concept of "monitoring", the authors have studied its functions, types and at the same time they made a brief overview of all ongoing controls monitoring in the field of circulation of medicines; analyzed the social function of monitoring and getting feedback from the General public and the consolidated opinion about the state of Affairs in the field of circulation of medicines in the eyes of the consumer.

CRITERIA FOR THE AFFORDABILITY AND QUALITY OF PALLIATIVE MEDICAL CARE IN REGULATORY LEGAL ACTS

Abstract
The functioning of the quality management system of medical activity is impossible without information about the quality of medical care and its affordability. For the purpose of objective evaluation in the scientific literature, it is proposed to define criteria with the subsequent selection of the most significant indicators for each criterion. On the territory of the Russian Federation, the criteria for the affordability and quality of palliative care are regulated by a number of regulations (both at the Federal and regional levels). This publication presents a simulation of the classification of the above criteria in accordance with the principles described in scientific sources. As a result of the study, it was found that the indicators are fixed in the normative field, since they assume an exact value expressed in different units of measurement; evaluation and comparison of a number of indicators causes difficulties due to the fact that they are expressed in absolute values; there are no criteria for directly provided palliative medical care.

Abstract
The aim of the investigation was to study the features of certification of medical graduates of the Institute of surgery at the Northern State Medical University. The analysis disclosed some methods and techniques that are used in terms of anti-epidemic measures in a pandemic coronavirus infection, presents problems of organization of distant substantive work of students in preparation for certification. Special emphasis is placed on the study of methods of interactive teaching of surgical disciplines, in particular simulation training. The features of conducting practical classes associated with independent work (patient supervision, writing a medical history, studying material for solving situational problems) are emphasized. The forms of teaching and quality control are systematically described. The advantage of using distance learning in the context of a pandemic has been proven.

CONTRIBUTION OF ACADEMICIAN OGANES BAROYAN IN THE DEVELOPMENT OF EPIDEMIOLOGY

Abstract
The scientific heritage of the Russian Epidemiologist, Health Care Organizer, Academician Oganes Baroyan (1906-1985) is still in demand today. The publication presents the main stages of the scientific biography and practical activities of the scientist. Countries are indicated on the territory of which the scientist took an active part in organizing and combating especially dangerous infections for many years. The assessment of Oganes Baroyan's work in Geneva as assistant Director General of the World Health Organization is given. The article describes the activities of the scientist as Director of the research Institute of Epidemiology and Microbiology named after N.F. Gamalei of the USSR Academy of medical Sciences, where he directed research on actual problems of medicine, the development of new promising directions for the development of epidemiology. The publication also notes the participation of O.V. Baroyan in the work of various ministries and departments of the USSR to localize and eliminate the outbreak of smallpox in Moscow (late 1959-early 1960).
