PROACTIVE RISK MANAGEMENT OPPORTUNITIES IN HEALTH CARE BY EXAMPLE OF ONCOLOGICAL PATIENTS

Abstract
The effectiveness of medical care, on the example of oncology, is determined mainly by the effectiveness of overcoming the barriers to accessibility of medical technologies (availability, affordability, acceptability, awareness, perception, elimination of administrative barriers). This process takes time, so the only effective way to realize the positive characteristics of medical technology is to plan in advance the volumes of medical care and the conditions that ensure its successful implementation. Proactive risk management in health care takes into account the results of monitoring and forecasting to determine both the need for medical care and availability of medical technology to assess and eliminate the risks associated with public health. The basis for effective monitoring and forecasting of public health is the use of correct statistical information based on a WHO family of international classifications, and the basis for choosing the most effective medical technologies is the forecast (“Horizon Scanning”) of the appearance and evaluation of medical technologies, as well as their optimal use based on clinical recommendations. At present, the main elements of the formation of expert analysis and decision-making mechanism have been created, allowing the use of proactive public health risk management.

Abstract
Modern health care around the world is increasingly faced with the problem of nosocomial infections, which lead to an increase in treatment costs, the duration of hospitalization and the deterioration of clinical prognosis for patients. The incidence of nosocomial infections is an indicator of the quality of care. The use of advanced technologies of modern bacteriological diagnosis, the interaction of physicians with clinical pharmacologists, epidemiologists and bacteriologists allows us to provide quality medical care and prevent the spread of nosocomial infections. Development and implementation of programs and strategies to curb the growth of nosocomial infections in Russia and abroad shows its effectiveness.

SCHOOLS FOR STROKE PATIENTS AND THEIR RELATIVES

Abstract
Introduction: Secondary stroke prevention includes several aspects, among them promotion of patient awareness of risk factors, symptoms of acute cerebrovascular accident (stroke) and actions needed in case of suspected stroke. Aim: to evaluate the results of work of schools for stroke patients and their relatives in primary stroke units and regional stroke center of Tyumen district. Patients and methods: Since 2012 schools for stroke patients and their relatives were implemented in primary stroke units and regional stroke center. Educational program includes 10 themes concerning stroke risk factors (hypertension, obesity, smoking, low physical activity and high level of stress), basics of primary and secondary prevention, main consequences of stroke (paresis, aphasia, pressure sores etc.) and ways of their correction. A special questionnaire was developed to evaluate the efficacy of educational measures, which included questions on gained knowledge and respondent opinion on the quality of education. An analysis of 2200 randomly selected questionnaires was performed. Results: A high level of attendance was demonstrated; most popular lectures included “Hypertension and stroke”, “Life after stroke” and “Stress and health”. Questionnaire survey revealed that educational materials were sufficient for most respondents (91.6%), easy-to-understand (97.3%), attendees acquired skills on stroke patient care (90.6%) and knew how to provide first aid in case of suspected stroke (98.8%). Conclusion: We recommend to continue educational programs for stroke patients and their relatives and questionnaire surveys to improve quality and efficacy of medical service.

AVAILABILITY OF INFORMATION ABOUT THE AMOUNT OF THE WASTED DONOR BLOOD

Abstract
We studied the validity of the refusal for a citizen to provide information on the results of screening for blood donor infections. It has been established that such a refusal leads to a violation of the patient’s right to information about the risk of medical intervention, as well as fraught with blood collection among contingents with a high prevalence of infections. It is concluded that it is necessary to replace the ban on the recommendation to widely publish such information.
