Experience of creating and using a computer program to optimize work with citizens' requests in the healthcare of the Altai Krai

Abstract
In the healthcare system, an important indicator reflecting the availability and quality of medical care is the rate of citizens' appeals to government bodies at all levels. Due to the expansion of openness of executive bodies, the expansion of channels for receiving citizens' appeals, and the growth of legal literacy of the population, their significant growth is noted. Receipt of citizens' appeals to government bodies regarding violation of their rights and legitimate interests is the basis for conducting unscheduled inspections as part of departmental control over the quality and safety of medical activities. For the effective detection and generalization of violations for any period of time and their timely suppression, as well as effective management decision-making, it is important to have the ability to conduct a detailed analysis of citizens' appeals in the context of medical organizations, including taking into account their validity (confirmed defects). The regional information system with the Subject Classifier of Appeals of Citizens of the Russian Federation did not allow for a detailed analysis of citizen appeals for each medical organization. In order to optimize work with citizen appeals, systematize the reasons causing complaints, improve the efficiency of management decisions by authorities in the healthcare sector and reduce the possibility of existing risks being realized, software was required to solve the issues raised. The results of testing the developed computer program based on an additional classifier of citizen appeals confirmed that it was possible to significantly optimize work with citizen appeals, ensuring the possibility of their detailed analysis in order to make effective management decisions in the healthcare sector of the region.

Stages of development of modern management information technologies in perinatology of the Tver region

Abstract
The article considers the possibility of using educational programs for children in health schools as an effective way to prevent exacerbations of chronic rhinosinusitis. The creation of health schools for children with chronic rhinosinusitis, which combine medical and social aspects, will contribute to the formation of healthy lifestyle skills in children and reduce the risk of acute rhinosinusitis becoming chronic.

Energy sector employees’ stress level assessment

Abstract
Introduction. Workplace stress is a considerable risk factor for the development of chronic non-communicable diseases (NCDs), such as cardiovascular diseases, diabetes, and obesity. Annual global economic losses due to workplace stress exceed 8 trillion dollars. This study aimed to assess the stress levels among employees in the energy sector of the Ural Federal District, working under acceptable and harmful labour conditions, and to analyse their satisfaction with personal and professional life. Materials and methods. In 2022—2023, 4653 employees from the energy sector were surveyed. The questionnaire included questions about stressful situations, personal and professional life satisfaction. Data were analyzed using statistical methods, including Student's t-test and Pearson's χ²-test. Results. Significant differences in stress levels and life satisfaction were found between groups working under acceptable and harmful conditions. Employees in harmful conditions more often reported complete satisfaction with personal and professional life (73.85% and 63.53% respectively in 2023), but also experienced more difficulties in coping with stress. In acceptable conditions, satisfaction levels decreased, while the frequency of stress remained high. Among different types of activities, the highest satisfaction was observed among operational staff, although a negative trend was noted in their assessments in 2023. Conclusion. Working conditions and type of activity significantly affect stress levels and life satisfaction. To reduce stress and improve employee well-being, it is recommended to implement stress management programs, improve working conditions, and regularly monitor stress and satisfaction levels. Limitations. The study is limited by geographical and sectoral factors, as it covers only employees in the energy sector of the Ural Federal District. Ethics. The study was carried out in compliance with the principles of the Declaration of Helsinki of the World Medical Assembly, adopted in 1964.

Practical use of a chatbot for frailty screening

Abstract
Purpose of the study. Epidemiological analysis of frailty among older adults using a chatbot on the Telegram messenger platform. Materials and methods: The analysis included 107 individuals aged 60 and older who completed the chatbot survey. The study assessed the prevalence of frailty, prefrailty, and specific geriatric syndromes within the sample, as well as parameters such as frequency of doctor appointments, number of hospitalizations, presence of chronic diseases, and the level of awareness and understanding of this condition among the population. Results: Frailty was presumed to be present in 15.9% of study participants, while probable prefrailty was identified in 22.4%. At the same time, at least one geriatric syndrome was observed in 86.0% of individuals aged 60 and above. Additionally, 50.5% of respondents had heard of the term “frailty”, but 59.8% misunderstood it, considering it a normal part of aging. Respondents from Moscow and St. Petersburg were more likely than others to correctly state that frailty syndrome is not a natural aspect of the aging process. Conclusion: The use of a chatbot improves the efficiency of data collection and processing for epidemiological analysis. The results obtained indicate that the prevalence data of the identified syndromes among respondents are comparable to those from statistical studies. Although half of the respondents are aware of this condition, their understanding of frailty syndrome is often inaccurate, which is a concerning trend that highlights the need to improve health literacy in the population. Therefore, the digital tool used in this study may serve as an effective method for conducting screening research and monitoring awareness of frailty syndrome and prefrailty.

Regulatory and Legal Framework for Financing Medical Care and Financial Reporting of Medical Organizations

Abstract
The issues of planning and financing medical care are inextricably linked to its accessibility and quality at all levels of the healthcare system. The availability of reliable and sufficient information on resource provision and its utilization directly affects the quality of managerial decisions, including those concerning the efficiency of medical organizations. Objective: To conduct an analytical review of the legal and regulatory framework regarding the planning and financing of medical care, as well as the formation of statistical and financial reporting by public medical organizations. Materials and Methods: Content analysis of regulatory legal acts and other documents concerning the regulation of planning and financing medical care at the federal level and the level of the constituent entities of the Russian Federation, as well as federal requirements for maintaining statistical and financial reporting at the level of medical organizations. Results: The conducted content analysis revealed that the planning and financing of medical care, as well as the principles and requirements for reporting in public medical organizations, are governed by a large number of regulatory documents of various statuses and levels. This complicates the process of monitoring and assessing the effectiveness of financial activities of medical organizations and hinders the search for ways to enhance their financial sustainability, including at the federal level where the basic planning and allocation of healthcare system resources take place. Conclusion: There is a need to develop new approaches for creating clear and comprehensible evaluation indicators and a methodology for federal level monitoring of the financial condition and performance efficiency of public medical organizations.

Effectiveness of professional retraining of mid-level specialists as a mechanism for the formation of multidisciplinary rehabilitation teams

Abstract
Increasing the life expectancy of citizens of the Russian Federation is the goal of the state programme «Development of Healthcare» [Resolution of the Government of the Russian Federation No. 1640 of 26 December 2017 On Approval of the State Programme of the Russian Federation ‘Development of Healthcare’]. The most important role in achieving the expected results is to improve the quality of life and reduce the disability of the population. One of the tools to fulfil these objectives is the improvement of medical rehabilitation measures. The principles of a multidisciplinary approach are applied to ensure a continuous recovery process. Formed multidisciplinary rehabilitation teams include specialists with higher and secondary professional education. Provision of structural units with mid-level specialists in the field of medical rehabilitation is carried out through retraining of medical personnel. The Department of Secondary Professional Education of the Federal State Budgetary Educational Institution of Postgraduate Professional Education RMANPO of the Ministry of Health of Russia in 2024 took part in the implementation of measures of the federal project ‘Providing medical organisations of the health care system with qualified personnel’ on professional retraining of mid-level specialists.

Social work in healthcare: from theory to practice

Abstract
Improving the quality of life of people with disabilities is directly related to the introduction of a multiprofessional approach, which determines the participation of specialists of various profiles, including specialists in social work, in health protection, including the provision of various types of medical and social assistance. The development of professional social work in Russia over the past thirty years has allowed them to expand the scope of their professional activities, one of which is the field of health protection. Professional social work in healthcare, in particular, in medical organizations, can be considered as a separate specific area of its activity — medical and social work. To date, theoretical, methodological, legal and organizational foundations of medical and social work have been developed, and personnel are being trained in the system of higher and secondary vocational education. At the same time, the country has only local experience of their participation in providing medical and social assistance to people with mental disorders, addicted to psychoactive substances, in need of palliative care, since issues remain unresolved related to both the «narrow» understanding of the essence of medical and social work by the medical community, awareness by doctors of the needs for such specialists, the introduction, for example, of a team method of work in medical organizations of specialists of various profiles, as well as issues of interdepartmental interaction between organizations and specialists of health and social protection systems, patients and recipients of social services in organizations that need an integrated approach to providing them with various types of medical, social and other assistance.

The impact of the information environment on the population's coverage of preventive vaccinations

Abstract
The article examines the impact of the information environment on the level of population coverage with preventive vaccinations in the context of the digitalization of society and the dissemination of false information. The key information barriers and factors of trust in sources of information about vaccination are described. The data from sociological and epidemiological studies, statistics on vaccination coverage in Russia and abroad are presented, as well as an assessment of the role of government policy and media support for preventive campaigns. The statements of experts in the field of immunoprophylaxis and the provisions of current regulatory documents regulating the field of vaccine prevention are analyzed.

The phenomenon of self medication: domestic and Foreign Experience

Abstract
Self medication is a global health problem that is becoming particularly relevant in the context of digitalization and limited access to medical care. In the Russian Federation, this practice is recognized as one of the key threats to the healthcare system, which requires the development of scientifically sound strategies to minimize it. The analysis of international and Russian experience has revealed the main factors of the spread of self medication: organizational barriers (difficulties in making an appointment with a doctor), economic constraints, low medical literacy (denial of the risks of self medication), and the availability of over the counter medications. The development of antimicrobial resistance and complications of diseases is particularly dangerous. Effective measures aimed at preventing and reducing the effects of self medication include: improving regulatory regulation, introducing educational programs, developing telemedicine services, and monitoring the advertising of medicines. The results of the study emphasize the need to move to the concept of responsible self medication, taking into account international experience and Russian specifics.

Organizational ways to improve the prevention of tobacco and nicotine-containing products consumption in pregnant women

Abstract
Despite the current level of medical technology, adverse pregnancy outcomes associated with preventable causes, such as maternal smoking and secondary exposure of the fetus to tobacco smoke through passive smoking, remain a serious public health problem. Purpose of the study: to substantiate ways to improve the prevention of tobacco and nicotine-containing product consumption in pregnant women. Materials and methods. We analyzed domestic and foreign studies on smoking during pregnancy conducted at the national, regional and global levels, time series of integral indicators of maternal and child health, approaches to predicting the degree of perinatal risk, as well as the age structure of women giving birth in the Russian Federation for 2010—2022 with the calculation of linear trend models. Results. In Russia, the proportion of pregnant women who smoked during pregnancy before delivery ranges from 15% to 23%. The share of traditional cigarettes accounts for 61.7% (95% CI: 52.2—70.5), smokeless products were used by 20.9% (95% CI: 14.1—29.7) and 17.4% were dual smokers (95% CI: 12.6—17.8). The majority (79.1%) of female smokers, whose average age was 29.5±5.2 years, are confident in the safety of smokeless products for the fetus and newborn. During 2010—2022, a trend of decreasing perinatal mortality and increasing maternal mortality was observed in the Russian Federation. The dynamics of the number of women giving birth in the age groups 15—24 and 25—34 were characterized by the same downward trend, but increased in the group of 35 years and older. Conclusion. Thus, timely detection and correction of tobacco and nicotine-containing product consumption by primary care health workers in women of childbearing age at the stages of pre-pregnancy preparation and pregnancy management, lactation and dynamic monitoring of the child can have enormous demographic and socio-economic significance.

The role of nurses in the prevention of cardiovascular diseases: literature review

Abstract
Cardiovascular diseases (CVD) remain the leading cause of death and morbidity worldwide, which requires effective prevention and control strategies. Nurses occupy a key place in the primary health care system, providing early detection and management of CVD risk factors. Their contribution to preventive measures is becoming especially important against the background of the growing number of patients with cardiovascular diseases. The aim of the study is to study successful nursing strategies focused on the prevention of cardiovascular diseases and risk factors for their occurrence in the population in the context of primary health care. Materials and methods. A systematic search of publications in open databases for 2020—2025 was conducted using keywords related to nursing and CVD prevention. Results. 3 key areas of nursing interventions in the prevention of CVD have been identified: 1) early detection of risks (systematic screening, application of standardized assessment scales, step-by-step monitoring of health indicators); 2) educational programs (individual and group counseling, development of personalized recommendations for lifestyle modification, motivational support of the behavior change process); 3) usage of digital solutions (remote monitoring of the patient's condition, the use of mobile platforms to support adherence to therapy, the use of telemedicine technologies for preventive counseling). Conclusion. The participation of nurses in CVD prevention programs has proven its importance for early detection and management of cardiovascular risks. Patient education and motivation, as well as integration into teamwork with doctors, contribute to sustainable improvement of health and reduction of morbidity. The use of innovative approaches and technologies expands the possibilities of prevention.

Training of medical workers in urgent ultrasound diagnostics technologies

Abstract
In the context of the rapid development of medical technologies and increased requirements for the quality of emergency care, the problem of training ultrasound diagnostics specialists to perform urgent examinations is becoming particularly relevant. The article examines the theoretical and applied aspects of the formation of professional competencies in the field of point-of-care ultrasound (FOCUS), including the differences between urgent and routine diagnostics, the specifics of educational strategies and the obstacles faced by practitioners. Recommendations on optimizing the system of continuing medical education, taking into account the tasks of emergency medicine, are presented.

Formation of postgraduate training of doctors at the Central Institute for Advanced Medical Education during 1936—1941 period

Abstract
This article is focused on analysis of postgraduate medical training forms at the Central Institute for Advanced Medical Education (TsIU in Russian) from 1936 (adoption of the new charter) until 1941 (evacuation from Moscow). Having received the status of head institution, TsIU became an educational and methodological center of postgraduate training of doctors all over the Soviet Union. In the prewar period (before World War II) preference was given mostly to courses of advanced medical education and courses of specialization along with individual on-the-job training. Annually, there was an increase in the number of students, clinical bases and chairs of the Institute.

Polar explorer and marine hygienist L. M. Starokadomsky (1875—1962)

Abstract
The article is devoted to L. M. Starokadomsky, a Russian naval physician and polar researcher, a prominent specialist in the field of hygiene of water transport. He participated in the development of the Northern Sea Route in 1910—15, and in 1932—34 in the North-Eastern polar regions. He participated in the North-Eastern polar expeditions of the People's Commissariat of Water Transport. An island he discovered in the Severnaya Zemlya archipelago (1913) was named after him. Author of manuals on hygiene of sea transport.

At the outset of Histology: Marie-François-Xavier Bichat

Abstract
In this publication, based on the study of original French literary sources, the stages of education, formation of scientific views and main achievements of the founder of histology — an outstanding French physician of the period of the French bourgeois revolution, Marie-François-Xavier Bichat (1771—1802) are analyzed; some inaccuracies regarding his biography and date of birth found in foreign and Russian literature are discussed and corrected; for the first time in the Russian historical-medical literature the list of twenty one tissues identified by him has been presented (in original French and with the authors’ translation into Russian); his contribution to the development of fundamental sciences — anatomy, histology, pathological anatomy (tissue pathology), as well as therapy and surgery is described; and tribute maintaining memory of him in his homeland is although shown.

Clinical use of blood in civil healthcare during The Great Patriotic war

Abstract
Today, the actively used method of transfusion of donated blood and its components, during the Great Patriotic War, due to its novelty, was not sufficiently mastered in civilian healthcare. By the beginning of the Great Patriotic War, transfusion had not yet become widespread in the USSR. In fact, the Great Patriotic War was the first serious test for the blood service. To meet the needs of the armed forces, a wide network of blood procurement was deployed with the involvement of a large number of civilians and military personnel of the rear units. In order to save money, various types of scrap blood began to be used in civil healthcare. The activities carried out proved their effectiveness, and due to the opportunities that transfusion specialists opened up, Soviet healthcare received a powerful impetus in a number of areas.

Physiological aspects of the use of gravidan in the treatment of malaria in the USSR in the 1930s

Abstract
The spread of malaria all over the world, the millions of victims it claimed, attracted the great attention of scientists from different countries. The deterioration of environmental and socio-economic conditions after the First World War and the Civil War significantly affected the spread of malaria. The late 20s and early 30s of the 20th century were marked by various biomedical experiments, including attempts to create a universal drug that increases overall life potential and quickly restores human performance and activity. An attempt was made to treat such a dangerous climate-dependent disease as malaria with drugs whose pharmacological action would not have side effects. One of these areas was the creation and experimental study, and then mass production, and the further use of gravidan in the treatment of malaria, which was of urgent importance at that time. Of course, today the use of this method looks exotic and unscientific, but this is a historical fact that took place in Russian medicine. The historical and medical aspect of its use in the treatment of malaria, both as monotherapy and in combination with specific antimalarial drugs, gave hope for some success and was of priority importance not only therapeutically, but economically. Thus, the drug gravidan, created in the USSR, entered the history of science as the first hormonal drug capable of activating the internal defense mechanisms of the human body, and thereby supporting its physical and cognitive well-being.

On the methodology of modern research on the history of medicine and healthcare in the USSR

Abstract
The narrative we inherited from the Soviet history of medicine and healthcare in the USSR is largely distorted or intentionally falsified. In order not to fall into error when researching this material, it is necessary to use three mandatory techniques: referring to primary sources, the method of criticism of the source and the comparative historical method. Another important methodological issue is the study of scientific schools in medicine and the criteria for distinguishing these schools (including moral criteria). The authors conclude that it is necessary to describe the history of medicine of this period anew.

History of the student scientific circle of the department of infectious diseases and epidemiology of the Russian university of medicine

Abstract
The trends of the era leave their mark on the format of the modern student scientific circle (SSC), which is gradually transforming into a «forge of personnel» not only for practical healthcare, but also for research organizations. The Order of the Government of the Russian Federation dated July 25, 2022 No. 2036 r «On approval of the Plan for the Decade of Science and Technology in the Russian Federation» emphasizes the involvement of schoolchildren and students in research work. This program is actively implemented by government in schools and universities. The Department of Infectious Diseases and Epidemiology of the Federal State Budgetary Educational Institution of Higher Education, Russian University of Medicine of Ministry of Health of the Russian Federation is no exception. On the basis of the department, in addition to the SSC, under the supervision of employees, students who do not attend the circle on a regular basis, conduct scientific work on the discipline of «infectious diseases». In the direction of «infectious diseases», schoolchildren are creating projects also under the auspices of the department. The above suggests that students' research work is gradually moving beyond the SSC, and the SSC itself may become more complex (or part of a more complex) structure. In order to improve the work of the SSC, it is necessary to recall its history and extract from it seemingly forgotten facts that can be useful both in terms of innovations and in accounting for errors. Knowing the history of the SSC will help students better connect with its «present» and understand their future roles as doctors, scientists, researchers and citizens.

Surgical smoke as a risk factor for the morbidity of scrub nurses in a professional environment

Abstract
The state of health of scrub nurses is influenced by professional activity. The article presents an analysis of the effects of surgical smoke on the health of operating nurses, the frequency of development of chronic oropharyngeal pathology and symptoms during work in the operating unit, the load on the scrub nurse per shift is presented. The study proved the direct and strong influence of surgical smoke on the occupational morbidity of scrub nurses, at the same time, directions for a preventive solution to the problem of exposure to surgical smoke were identified and presented.

Organization of measures for the prevention and control of malaria among the population of the Saratov region during the Great Patriotic War

Abstract
The article is devoted to the activities of health care authorities, malaria and sanitary epidemiological stations and the House of Health Education of the Saratov Region in organizing and conducting comprehensive measures, including active detection of malaria patients, diagnosis, therapy and prevention of the disease, among residents of the region and military personnel who were treated in evacuation hospitals. During the Great Patriotic War, malaria and sanitary control points were additionally organized in endemic areas of the region to carry out large-scale measures to prevent malaria, medical workers of which regularly gave lectures, gave talks, accompanied by exhibitions of posters and the distribution of leaflets and brochures. All materials on sanitary and educational work for doctors and paramedics were prepared by the staff of the House of Health Education. The paper presents an analysis of the incidence of malaria in the region for 1941—1944 and the multifaceted activities of all links of the medical service aimed at stabilizing, reducing and improving the epidemiological situation in the region.

An analysis of migration processes (number/ratio of arrivals, departures and migration balance) for the period 2000—2022 was conducted. in the Republic of Sakha (Yakutia), which is one of the regions of the Federal District of Sakha (Yakutia) and 13 districts of the Arctic zone of Russian Federation. The regions with whom the most frequent migration «exchange», the age period and the educational census of the most active migrants are specified. In addition, the areas from which the most leave and those where the most arrive are identified. The presented tables show the peculiarities of migration processes depending on medical and economic zoning. It turned out that the most attractive city for moving is the capital of the republic — Yakutsk. It becomes obvious the need to strengthen the attention of the leadership of the republic and municipalities to migration processes, to create conditions for a positive balance of migration, consolidation of the population and, especially, the working age, to improve the demographic situation on the ground.

Analysis of indicators and structure of primary disability in the Republic of Dagestan for 2022—2023

Abstract
According to the World Report on Disability provided by the World Health Organization, there are more than one billion people living with some form of disability in the world, which is about 15 per cent of the world's population. In Dagestan, according to the Ministry of Health of the Russian Federation, there is a high proportion of disabled people among the adult contingent and children with disabilities from the total number of disabled people. The analysis of disability and its structure makes it possible to make management decisions on the prevention of disability, consisting in improving the quality and accessibility of rehabilitation measures. In addition, it requires measures to increase social programs and medical and social activities that promote the integration of people with disabilities into social processes. The trend of growth of primary disability in the Republic of Dagestan, the need for medical and social services of the republic to obtain scientifically based information regarding the disability of the adult population of the republic and the need to develop scientifically based measures to reduce morbidity and disability determine the relevance of this article.

Development of proposals for improving regulatory framework and implementation of federal projects in healthcare at the regional level

Abstract
The objective of this study was to develop proposals for improving the legal framework and implementation of federal projects in healthcare at the regional level. The following key proposals were formulated and scientifically substantiated: development and approval of unified methodological approaches to the development of «core» indicators and activities of federal projects in healthcare; inclusion in federal projects of only those activities for which an assessment of clinical and economic effectiveness was carried out; development and implementation of a system for detailed analysis (monitoring) of the actual results of the implementation of federal projects over the past years; creation of a «flexible» mechanism for making changes to the federal project passport for the purpose of prompt «fine-tuning» of activities and indicators; increasing the role of «supporting» activities of federal projects (provision of patients with drugs and medical products) and activities aimed at improving the effectiveness of medical care and the efficiency of the organization of medical care. Also, in the course of this study, proposals were formulated for assessing the risks of implementing current federal projects in healthcare at the regional level for the period up to 2030.

Skin diseases among nurses and medical college students as a risk factor in healthcare systems in various countries

Abstract
The professional activity of health workers is characterized by the complex impact of multifactorial risks, including biological, psychosocial, ergonomic and chemical threats. Increased susceptibility of nurses to the risk of developing occupational contact dermatitis is noted, which is due to the peculiarities of their professional activity. The prevalence rate of this disease in this professional group, according to various authors, ranges from 7% to 46%. Such fluctuations can be caused by both objective factors, such as the intensity of the work process and the availability of personal protective equipment, and subjective ones, including individual characteristics of skin reactivity and compliance with hand care recommendations. A direct relationship has been found between the development of dermatological problems and a decrease in performance, which negatively affects the overall functioning of a medical organization. It is believed that nursing students are at high risk of developing occupational skin diseases. This pathology occurs in 20.9% of nursing students.

The role of environmental determinants in the formation of public health (literature review)

Abstract
Introduction. This review article discusses the place and role of environmental determinants in public health from different perspectives. Purpose of the study. To study the role of environmental determinants in public health. Material and methods. In preparing the review, a reporting method was used for systematic reviews and meta-analysis. The search was carried out in the bibliographic databases Elibrary, PubMed, MEDLINE, Google Scholar for the keywords «environmental determinants, public health, environment, health risk factors». Results. The article deals with the categories and factors that determine the environmental determinant from the standpoint of public health. Various methods for assessing the impact of environmental determinants on public health are presented. Their strengths and weaknesses are considered. Based on the results of the analysis of methods for assessing the impact of the environmental environment on health, it was proposed to take into account the combined and combined impact of environmental factors, as well as the time factor, the characteristics of gender and age influence. Conclusion. The concept of the term “environmental determinant” and the categories of parameters that determine it differ significantly in studies. From the standpoint of a deep understanding of ecology as a science and public health, an environmental determinant is the environment surrounding a person in all its manifestations, covering all spheres of life and affecting his health. There is no single methodological approach to assessing the impact of environmental determinants on public health. Many methods have certain disadvantages and are of limited use. To assess the impact of environmental determinants on public health, it is necessary to make a purposeful choice of indicators that fully meet the goals and objectives of the study. With monitoring of environmental determinants by government in all subjects of the Russian Federation, both operational and planned impact on public health is possible, taking into account newly emerging and chronic environmental factors.
